Acknowledgment of Risk:
I, the undersigned parent/guardian of the above-named camper, acknowledge that participation in Camp ACTIVATE involves risks, including but not limited to physical injury. I understand these risks and agree to assume full responsibility for any injuries or damages that may occur during the program.
Release of Liability:
In consideration of my child’s participation in Camp ACTIVATE, I hereby release and discharge the program, its staff, volunteers, and the Greater Lansing Islamic School from any and all claims, demands, or causes of action that may arise from participation in this program, whether caused by negligence or otherwise.
Medical Authorization:
In the event of an emergency, I authorize the Camp ACTIVATE staff to seek medical treatment for my child. I understand that I will be notified as soon as possible.
Photographic Release:
I give permission for photographs and/or videos of my child to be taken during Camp ACTIVATE and used for promotional purposes.
Agreement:
By agreeing, I acknowledge that I have read and understood this waiver and release of liability. I agree to the terms and conditions set forth herein.
